SUPPLIES
I used photoshop cs6 but any version should do.

Font of choice
Let's get started!
Open template and apply the papers to the layers listed below.
To do this just place the paper over the layer and hit ctrl + alt + G
and this will create a clipping mask.
After you have your paper positioned the way you like go
ahead and merge it to the layer and add a drop shadow.
1 - Paper5 - Merge.
2 - Add Gradient of choice.
3 - add design of choice.
4 - Paper12 - Merge.
5 - Paper12 - Merge.
6 - Paper5 - Merge.
7 - Add style of choice.
8 - Paper12 - Merge.
9 - Add Gradient of choice.
12 & 13 - add design of choice.
14 & 15 - add design of choice.
16 & 17 - Add Gradient of choice.
Open NotePad.
Duplicate and place one of layer 10
and the other on layer 11.
Use the clipping mask the same way you did with your papers.
Rotate Notepad around and get it how you like then Merge.
Open up a close up of your tube and duplicate.
Place one on 10 and mirror on 11.
Now open up close up 2.
Place over layer 2 and change blend mode to Luminosity.
Make the eye peak over the top of template.
Erase the bottom then duplicate close up
and flip horizontal.
Make so that you see the eye peaking out of the bottom half.
See tag for exact placement.
Now lets add some elements!

FenceWithBlood
Ribbon1
Feel free to add more or totally different elements.
Make it personal to you.
Open your mask and apply paper.
I used Paper1 on mine.
Add your name/quote and copyright.
Add any desired filters, save and you're done!
